Cancellation Request

Cancellation Policy: 

If you wish to terminate your membership, we require you to do so at least 7 business days before the last day of the month.  We require all cancellations in writing by submitting the following form.  If you fail to cancel in writing within the required 7 business days before the last day of the month, you will be liable for the next month’s payment.  There is a $25 cancellation fee when membership is terminated. 

Once the request form is submitted, you will receive a confirmation of the cancellation within 2 business days (M-F).  Please do not assume your membership is canceled – if you do not receive a confirmation by email, please call our office at 701-456-2074.
